FDA's pathway for regulation of FMT: not so fraught

چکیده
A recent article byRachel Sachs andCarolynEdelstein in the Journal of Lawand the Biosciences1 on regulation of fecal microbiota transplants (FMT) brought a welcome hard look at the difficulty of regulating an area that holds great promise but is often greeted by hoots and nervous laughter rather than comprehensive study. FMT has been given relatively little attention by legal scholars working in food and drug law. However, FMT and thebroader fieldof thehumanmicrobiome2 have enormouspotential for thedevelopment of new therapeutics.3 Already, FMThas been shown tohave greater efficacy for the treatment of recurrent Clostridium difficile infections (CDI) than traditional treatmentwith abroad spectrumantibiotic, vancomycin.4 Interventions thatmanipulate the human microbiota may also hold potential for the treatment of Inflammatory Bowel Disease5 and potentially a broader range of inflammatory, infectious, and metabolic diseases. Sachs and Edelstein take issue with FDA’s current approach to regulating FMT. Ensuring the safe and effective FDA regulation of fecal microbiota transplantation
Scientists, policymakers, and medical professionals alike have become increasingly worried about the rise of antibiotic resistance, and the growing number of infections due to bacteria like Clostridium difficile, which cause a significant number of deaths and are imposing increasing costs on our health care system. However, in the last few years, fecal microbiota transplantation (FMT), the tran...
متن کاملGestational diabetes leads to down-regulation of CDK4-pRB-E2F1 pathway genes in pancreatic islets of rat offspring
Objective(s): The link between a hyperglycemic intrauterine environment and the development of diabetes later in life has been observed in offspring exposed to gestational diabetes mellitus (GDM), but the underlying mechanisms for this phenomenon are still not clear. Reduced β-cells mass is a determinant in the development of diabetes (type 1 and type 2 diabetes). Some recent studies have provi...
متن کاملFeces transplantation for recurrent Clostridium difficile infection: US experience and recommendations
P atients who have failed to respond to repeated antibiotic treatment for recurrent Clostridium difficile infection (RCDI) present a particularly difficult challenge. Recent investigations of patients with RCDI have demonstrated significant disruption of the intestinal microbiome diversity as well as bacterial richness (1). Following the initial report on fecal microbiota transplantation (FMT) ...
متن کاملInactivation of mitogen-activated protein kinase signaling pathway reduces caspase-14 expression in impaired keratinocytes
Objective(s):Several investigations have revealed that caspase-14 is responsible for the epidermal differentiation and cornification, as well as the regulation of moisturizing effect. However, the precise regulation mechanism is still not clear. This study was aimed to investigate the expression of caspase-14 in filaggrin-deficient normal human epidermal keratinocytes (NHEKs) and to explore the...

would like to express my appreciation to the I American College of Chest Physicians for inviting me to join you here today, to discuss certain issues bearing on the Food and Drug Administration and its performance. Certainly, we have many things to discuss. For example, of all I heard about the so-called 'drug lag" when I was in Washington, I heard most about beclomethasone and the apparent slo...
